There are Three Ways to Register for the Conference:

  1. Online

  2. Download the Registration Form
    Click here to download the form, which can be e-mailed, faxed or mailed. See bottom of form for more information. Registrations received after December 1, 2010 will be processed on site.
  3. On site - Visit the ECCU Registration Desk during the following days/times:
    Tuesday, December 7 7:00 a.m. - 5:00 p.m.
    Wednesday, December 8 7:00 a.m. - 7:30 p.m.
    Thursday, December 9 6:00 a.m. - 5:00 p.m.
    Friday, December 10 6:30 a.m. - 5:00 p.m.
    Saturday, December 11 6:30 a.m. - noon

(Registrations processed on site may cause a delay at the time you check in at the registration desk. If you need to register after December 1, please bring your registration form and payment directly to the conference.)

Payment
Make checks payable to the Citizen CPR Foundation (CCPRF) in U.S. funds. Credit card payment is acceptable with a valid MasterCard, Visa or American Express. Authorized government purchase orders are accepted.

International Registration and Payment
If you would like to use a credit card, you must print the registration form and fax it to the number on the form. No online international credit cards are accepted at this time. If you are paying by check, you may register online, select the "pay by check" option and mail in your check.

Confirmation
For those registrations received prior to December 1, 2010, CCPRF will send a confirmation e-mail. When you receive your confirmation, please check the spelling of your name and the events for which you have registered to ensure that they are correct. If there is an error, please contact the CCPRF registrar at (703) 538-3548. The information in your confirmation e-mail will be used for your name badge and event tickets. If you do not receive a confirmation, please contact the registrar to confirm receipt of your registration. Please note that registrations received by mail or fax may take a few days to process.

Badges/Conference Information
For those attendees registering in advance, your badge and registration materials will be held for you at the CCPRF Registration Desk.

Cancellations and Refunds
Written notice of cancellation received on or before November 5, 2010 will be fully refunded. Written notice of cancellations received from November 6, 2010 through December 1, 2010 will be refunded less a $100 processing fee (to be processed after the conference). No refunds will be issued after December 1, 2010. Substitutions of registrants are allowed.
